
How to Apply In Person
If you would like to apply for housing outside of our programs for chronically homeless men and women, you will need to pick up an application from our main lobby at either location (Maxwell Garden or Maxwell Terrace) or print out the application.

Applications must be submitted in person. We cannot accept emailed or faxed applications

There is a $25 application fee per adult. Potential residents must meet income restrictions and provide proof of homelessness to apply. Restrictions are subject to change
Paperwork Needed
Along with the application, below is a list of paperwork you may need to bring with you to apply for housing.

Verification of homelessness: This can be a letter from a shelter, family member, friend, case manager or a personal written statement.

ID: Each potential resident must have ID. Types of ID accepted: Social Security Card, ID or Driver’s License. Birth certificate only accepted for children.

Income verification: Social Security letters must be current and dated within 90 days. Employer’s phone number, fax number, and address. Six weeks of most recent pay stubs. If no pay stubs are available, employer contact information suffices. If you collect income from other sources, please bring the most recent paperwork.

Asset verification: Must have proof of all assets. Direct Express, Check Card or Pay Card (as bank account) bring balance inquiry from ATM as proof of deposit. *we cannot accept a withdrawal slip, only a balance inquiry. Checking account statements (most recent 6 months). Savings account statements (most recent statement – 1 only).
